Please download the two PDF forms you will need for your Surgicare Appointment with Dr. Fulkerson.  Please have the physician’s office fill out the Surgicare History and Physical Form and fax to our office at least 2 days before the scheduled surgery.  Also, bring this form with you the day of your surgery.  The Post Op Instructions are for you to comply with after your surgery to assure optimum recovery.
